What should I do before I visit?

  • Plan to arrive 15 minutes early to your first appointment.
    • If you’ve never been to our clinic, we highly recommend if possible stopping by any day before your first appointment to do a test run and make sure you know exactly where we are located.
  • We located on the second floor directly above CHASE Bank and across the street from the Silver Cloud Hotel.
  • Elevator access can be found on the far west end of the building near Spa Envy (directly below Trailside Dentistry) and the access code is 20106#. Please record the access code in an easy to find location prior to your arrival.
  • Make a list of questions that you want your physical therapist to answer.
  • Gather your insurance card and photo ID.
  • Make sure our front office staff has helped you verify your insurance benefits.
  • Our front office team will email and text you a link to fill out the online paperwork ahead of time. If you don’t get that link within 2 hours after speaking to our front office team, please give them a call back and request for it to be resent. We strongly encourage you to complete this process as it will allow you to spend the maximum amount of time with your physical therapist.
    • Don’t want to wait? Use this link to get started on the forms. Make note that you will need to enter an email or phone number and your date of birth in order to get a 2 factor authentication code to proceed with completing the online paperwork.

What can I expect from my first visit ?

  • You will be greeted by our friendly front office staff who will help you complete any unfinished paperwork and get your checked in so your physical therapist knows you’ve arrived. Please have your insurance card and photo ID handy.
  • Your physical therapist for the initial evaluation will be out to greet you on time and will prioritize two things:
    • Letting you know the cause of your issue.
    • Laying out what your successful treatment plan looks like.
  • There should be time for treatment on the first day to get you started on your road to recovery. This treatment can consist of hands on techniques you cannot do on your own, key exercises and education on what to avoid/reduce or do more of in order to execute a successful plan.
  • When the evaluation and first treatment is done, your physical therapist will guide you where you need to go to schedule your follow up visits with one of our convenient self-service kiosks or with one of our friendly front desk team members. The specific number of visits you need to experience significant improvement will be prescribed by your therapist and we strongly encourage you to schedule out all of these visits before leaving the clinic to reduce the chances of those spots being taken.
  • If you have any questions at any time, please don’t hesitate to call. We want to ensure your satisfaction and full recovery.

Is there a no show/cancellation fee?

When we are not able to fill an appointment you need to cancel due to less than 24 hours notice, our fee of $65 helps to cover about 50% of what our company needs to break even on the visit. We prefer not to charge this fee but need to in order to cover costs such as rising rent and wages. We appreciate payment prior to or at the start of your next appointment.
