As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.

For those patients who do not have health insurance or those who have maxed out their benefits, we offer a private pay plan.  Please contact Kaizen Physical Therapy today for more information on our fees for service and payment details.

Motor Vehicle Cases – Fee Schedule

• This is what Kaizen Physical Therapy charges per commonly used CPT/treatment code and motor vehicle companies typically reimburse physical therapy companies for the full charged amount. This can give you a rough estimate of how much PIP funds you will use for each treatment session. However, it is not guaranteed that your physical therapist will select the exact same treatment codes for each session. Your first session/evaluation will always include 97161, 97162 or 97163 based on the physical therapist’s determination of the complexity of your case.
