Feb 18th 2020Please have your referral (if you have one) and insurance card ready when you call to schedule. This will allow us to verify your insurance benefits and provide you with an estimate of coverage.
Bring to your first visit:
- A prescription or referral if you have one.
- Insurance information (we will need a copy of your insurance card).
- In the case of an automobile accident or worker’s compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number.
I don’t have insurance – what do I do?
